If your document was not issued by a Florida government office, it must first be notarized by a Florida notary public using a proper English notarial certificate (acknowledgment or jurat). The Florida Department of State provides guidance and sample formats:

School Records (transcripts, diplomas, letters) Must be properly notarized, either the originals with an official school statement and notarial certificate, or notarized copies signed by the document holder with a complete notarial certificate.
